Background

Finasteride (brand name Propecia) is a medication used in the treatment of benign prostatic hyperplasia (BPH) and male pattern baldness. It was the first and only treatment to be approved for the treatment of male pattern baldness by the United States Food and Drug Administration (USFDA). Propecia has been shown to have fewer side effects in comparison to other hair loss treatments and is less likely to cause significant hair loss in some men. However, it is not known whether it can treat hair loss in the long term.

Methods

The aim of this study was to determine whether Propecia can cause significant hair loss in men with androgenetic alopecia (AGA) or pattern baldness.

Data source:This retrospective cohort study included all patients who received finasteride treatment at the Department of Osteopathic Medicine of the University of Liverpool NHS Trust in Liverpool. Patients had received finasteride treatment at the Institute of Osteopathic Medicine at the University of Liverpool for 5 years and the hair loss was determined using a self-reported clinical assessment. The patients were followed for 6 years to determine whether hair loss had been observed and the results of the hair loss measurement were considered as being representative of the pattern hair loss. A total of 1079 patients were included in the study. The data were collected from patients who were diagnosed with AGA or pattern baldness at baseline and the results of the hair loss measurement were considered representative of the pattern baldness. The mean age of the study population was 55.8 years (95% CI, 63-63.6). The mean duration of hair loss was 17.6 years (95% CI, 17.3-21.7). The mean number of hairs was 4.3 (95% CI, 3.2-5.6) and the mean age was 52.8 years (95% CI, 49.4-54.4). There was no significant difference between the patients in terms of hair loss in the groups of men with androgenetic alopecia and those without (p=0.824). Propecia showed no significant difference in hair loss between patients with androgenetic alopecia and pattern baldness. However, there was significant difference in the mean number of hairs in the group of men with androgenetic alopecia (p<0.01).

Conclusion

Finasteride can cause significant hair loss in men with androgenetic alopecia (AGA) or pattern baldness. It may be considered in the management of AGA or pattern baldness. Propecia should not be used in patients with male pattern baldness and is less likely to cause significant hair loss in men with AGA or pattern baldness.
